The Programme

The Heart Manual programme both Post MI and Revascularisation has three components

1. Heart Manual facilitator training

The programme is delivered to patients by health care professionals - usually nurses – who have completed the Facilitator Training Course, a practical two day introduction to the Heart Manual. The course covers the psychological and clinical background to rehabilitation, administering the programme, working with patients, and monitoring progress. For more information, click here.

2. Heart Manual facilitation

Patients, and when possible their partners, families and carers, are introduced to the Heart Manual programme, (book and audiotapes/CD) by the facilitator either in hospital or soon after discharge. The facilitator spends approximately one hour with the patient at this stage. The facilitator contacts the patient by telephone or visit at 1, 3 and 6 weeks to discuss progress using a semi-structured interview. At the end of the programme the patient completes a final questionnaire to assess the need for further intervention by the clinical team involved and to evaluate the success of the Manual.

3. The Heart Manual book

Both books are attractive guides, written in simple language and laid out in an easy-to-follow format. Briefly, the contents of the book comprise:

Part 1: Your Heart Attack/ Your operation : the Facts -Essential information for patients recovering in hospital.

Part 2: The Weekly Programme - In six weekly sections, a phased programme of home based exercises, health education, risk factor reduction and stress management.

Part 3: Facts and Advice to Help Your Recovery - Extra information and answers to frequently asked questions – about medicines, tests and revascularisation procedures; anxiety, stress, depression and further chest pains.

The audiotapes/CD:

Tape 1/CD contains a programme of relaxation exercises.

Tape 2 /CD (Post MI package only) contains scripted doctor/patient and doctor/carer interviews.
