The Heart Manual is a highly evidenced self management resource for patients with a cardiac condition and the families who support them. It is facilitated by specially trained health professionals UK wide (and internationally). The model has been used to develop a stroke version and its educational approach integrated into a telehealth infrastructure being built by Philips and funded by the EU.
The Heart Manual Programme (NHS Lothian) is the UK's leading home-based supported self management programme for individuals' recovering from acute Myocardial Infarction. As an NHS resource since 1992, it continues to provide an evidence based approach, assisting the recovery of 18,000 patients annually in the UK and abroad. The Heart Manual Revascularisation edition for patients who've had coronary artery bypass surgery (CABG), or angioplasty (PTCA/PCI) was launched by NHS Lothian in 2007 following a successful 4 year RCT with the University of Birmingham.
Both programmes are facilitated by specially trained clinicians-to enable patients’ consensus, understanding and management of their condition.
